Summit Counseling LLCprovides employee discounts to the employees of participating Waukesha County Business Alliance member companies. The discount also applies to the employee’s immediate family members (spouse and children under 25).
A discount of $10.00 off the current standard rate applies. The current standard rate is posted on the Summit Counseling web site Rates page. The discount applies to all visits as long as you remain an employee.
Some Healthcare Savings Account (HSA) cards pay for counseling services. The discount will apply if you are using your HSA card. The cards that have included counseling are:
- Associated Bank HSA Plus
- Optum Bank
- TASC
Summit Counseling does not accept insurance at this time. We do however maintain affordable rates so that our services are accessible to everyone. We accept cash, checks, Visa, MasterCard, Discover, American Express and PayPal.

Appointment reminders are sent automatically. A reminder will appear in your email in-box one week before your appointment. You will also receive a text message reminder the day before your appointment. If these reminders will cause a problem, please call to set up your appointments and request the reminders not be sent. Do not use the online appointment system.

The information allows the therapist to get up to speed quickly on the issues troubling you. Completing this form also helps you think more specifically about what you want to discuss with the therapist. Use only the SAMPLE Employee Intake Questionnaire to assure you that your discount will be applied.
